Description
Norditropin SimpleXx is a recombinant human growth hormone (rhGH) biosynthetically identical in sequence to endogenous pituitary growth hormone. It is indicated for the following, classified by strength of evidence:
Established / Approved Pediatric Indications
- Growth hormone deficiency (GHD) in children due to inadequate endogenous growth hormone secretion.
- Turner syndrome – short stature associated with this condition, confirmed by chromosomal analysis.
- Prader-Willi syndrome (PWS) – to improve growth and body composition, confirmed by genetic testing.
- Small for gestational age (SGA) – children born small for gestational age who fail to show catch-up growth by 2 years of age.
- Growth failure due to chronic kidney disease (CKD) – used up to the time of renal transplantation, in specific approved products.
- Idiopathic short stature (ISS)/Noonan syndrome-associated short stature – in selected products, where height is significantly below normal and no other cause is identified.
- SHOX gene deficiency – short stature due to short stature homeobox-containing gene deficiency (product-specific approval).
Established / Approved Adult Indications
- Adult growth hormone deficiency (AGHD) – either childhood-onset GHD confirmed in adulthood, or adult-onset GHD due to pituitary/hypothalamic disease, pituitary surgery, radiation therapy, or head trauma, confirmed by appropriate biochemical (stimulation) testing.
Other Recognized Use (Product-Specific / Guideline-Supported)
- HIV-associated wasting/cachexia – to increase lean body mass, for specific approved formulations, as an adjunct to overall management, not a substitute for antiretroviral therapy.
Norditropin SimpleXx is not approved for anti-aging use, athletic performance enhancement, bodybuilding, or growth promotion in children with normal, closed growth plates, and such use is inappropriate and unsafe.
Composition
Each vial, cartridge, or pre-filled pen of Somatropin contains synthetic human growth hormone produced by recombinant DNA technology, structurally identical (191 amino acid sequence) to human pituitary-derived growth hormone. Strength is expressed in milligrams equivalent to international units (approximately 1 mg ≈ 3 IU), commonly available as 5 mg, 5.3 mg, 6 mg, 10 mg, or 12 mg per vial/cartridge, depending on the brand and presentation.
Common excipients include glycine, mannitol, sodium phosphate buffer, poloxamer 188, and (in multi-dose liquid presentations) a preservative such as phenol or m-cresol; lyophilized (powder) presentations are supplied with a separate diluent for reconstitution.
Description
Norditropin SimpleXx is the biosynthetic (recombinant) form of human growth hormone (somatotropin), produced using recombinant DNA technology in bacterial (E. coli) or mammalian cell expression systems. It has the same 191-amino-acid sequence and biological activity as growth hormone secreted naturally by the anterior pituitary gland.
Norditropin SimpleXx is used as replacement or supplemental therapy in children and adults who cannot produce sufficient natural growth hormone, or in certain genetic and growth-failure syndromes where growth hormone therapy has been shown to improve linear growth, body composition, and metabolic parameters. It is administered by subcutaneous injection and requires ongoing supervision by a physician, typically an endocrinologist.
Therapeutic Class
Pituitary hormone; Recombinant human growth hormone (rhGH) analog. Norditropin SimpleXx belongs to the growth hormone / somatotropin class of biologic therapeutic proteins.
Pharmacology
Mechanism of Action
Somatropin binds to the growth hormone receptor (GHR) on the surface of target cells (primarily hepatocytes, chondrocytes, and adipocytes), inducing receptor dimerization and activation of the JAK2–STAT5 intracellular signaling pathway. This stimulates hepatic and peripheral production of insulin-like growth factor-1 (IGF-1), which is the principal mediator of Somatropin’s growth-promoting effects on cartilage and bone (linear growth in children).
Somatropin also has direct metabolic actions: it stimulates protein synthesis and nitrogen retention (anabolic effect), promotes lipolysis and reduces fat mass, and exerts an anti-insulin (diabetogenic) effect that can increase blood glucose, particularly at higher doses.
Pharmacokinetics
Following subcutaneous injection, Somatropin is absorbed with an absolute bioavailability of approximately 70–90%; peak serum concentrations occur roughly 3–6 hours after dosing. It is metabolized primarily in the liver and kidney, with an elimination half-life of approximately 2–4 hours after subcutaneous administration (longer than after intravenous dosing due to slow absorption from the injection site).
Dosage & Administration of Norditropin SimpleXx
Norditropin SimpleXx dosing is highly individualized by indication, body weight, growth response, and IGF-1 levels, and must be determined and monitored by a physician experienced in growth hormone therapy. See the Dosage table for indication-specific doses and the Administration field for injection technique. Norditropin SimpleXx is given by subcutaneous injection, typically once daily, with injection sites rotated systematically (e.g., abdomen, thigh, buttock) to reduce the risk of lipoatrophy.
Administration of Norditropin SimpleXx
Norditropin SimpleXx is administered by subcutaneous injection only, using a fine needle, pen injector device, or needle-free injector as prescribed. Practical points:
- Inject into subcutaneous tissue of the abdomen, thigh, or buttock; rotate the injection site with each dose to reduce the risk of lipoatrophy (localized loss of fat tissue) and lipohypertrophy.
- Reconstituted or liquid solutions should be inspected visually before use; do not use if the solution is cloudy, discolored, or contains particles (see Reconstitution).
- Injections are typically given once daily; for growth-promoting indications, evening dosing is commonly recommended to mimic natural nocturnal growth hormone secretion patterns, though the physician’s specific instructions should be followed.
- Patients and caregivers should be trained in proper injection technique, storage, and sharps disposal by a healthcare professional before self-administering at home.
- Do not share pens, cartridges, or needles between patients, even if the needle is changed.
